ABOUT SH EARCARE

I decided to set up this business in 2020 after noticing more and more people being turned away from GP Surgeries as they no longer provide an ear irrigation (ear syringing) service. I am a Registered Nurse with qualifications in Ear care and have worked in the community setting for over 20 years. I will be providing a quality service to clients who are at the heart of everything I do by using evidence-based practice to inform my decision making. 

At SH Earcare I provide Ear Irrigation Services (ear syringing) to Clients in the community. You can have your ears assessed and irrigated as necessary either at my place of work or in the comfort of your own home. I cover Blyth, Northumberland, North Tyneside and surrounding areas.

Both ears assessed with otoscope and evidence-based advice given. Allow around 10-15 minutes for this service.

Cost £30

Both ears assessed and one ear irrigated as necessary using the innovative Guardian irrigation machine to gently release the wax from the Tympanic Membrane (Ear Drum). Please allow up to 30 minutes for this service.

Cost £40

Both Ears assessed and irrigated as necessary using the innovative Guardian irrigation machine to gently release the wax from the Tympanic Membranes (Ear Drums). Please allow up to 45 minutes for this service.

THESE TERMS AND CONDITIONS ARE THE STANDARD TERMS FOR THE PROVISION OF SERVICES BY SH EARCARE, A SOLE TRADER.


EACH REFERENCE IN THESE TERMS AND CONDITIONS TO “WRITING” AND ANY SIMILAR EXPRESSION INCLUDES ELECTRONIC COMMUNICATIONS WHETHER SENT BY E-MAIL, TEXT MESSAGE OR OTHER MEANS.


INFORMATION ABOUT US

SH EARCARE IS A SOLE TRADER.

WE ARE A MEMBER OF THE NURSING AND MIDWIFERY COUNCIL (NMC) AND HAVE A CODE OF PROFESSIONAL CONDUCT. WE HAVE ROYAL COLLEGE OF NURSING (RCN) CURRENT MEMBERSHIP. WE ALSO HAVE PUBLIC LIABILITY INSURANCE WHICH IS AVAILABLE ON REQUEST.


APPOINTMENTS

ALL APPOINTMENTS FOR SERVICES MADE BY YOU WILL BE SUBJECT TO THESE TERMS AND CONDITIONS.

YOU MAY CHANGE YOUR APPOINTMENT AT ANY TIME BEFORE WE BEGIN PROVIDING THE SERVICES BY CONTACTING US.

IF YOUR APPOINTMENT IS CHANGED BY US, WE WILL INFORM YOU NO LESS THAN 48HRS OF ANY CHANGE.

YOU MAY CANCEL YOUR APPOINTMENT AT ANY TIME UP TO 48HRS PRIOR TO YOUR APPOINTMENT TIME WITHOUT CHARGE. IF YOU WISH TO CANCEL THE SERVICES AFTER THIS TIME PERIOD YOU WILL BE CHARGED £15.00.

WE MAY CANCEL YOUR APPOINTMENT AT ANY TIME BEFORE WE BEGIN PROVIDING THE SERVICES DUE TO THE UNAVAILABILITY OF REQUIRED PERSONNEL OR MATERIALS, OR DUE TO THE OCCURRENCE OF AN EVENT OUTSIDE OF OUR REASONABLE CONTROL.  IF SUCH CANCELLATION IS NECESSARY, WE WILL INFORM YOU AS SOON AS IS REASONABLY POSSIBLE. CANCELLATIONS WILL BE CONFIRMED IN WRITING/PHONECALL.

The Purpose of processing Client Data

In order to provide ear care treatments, I will need to gather and retain potentially sensitive information about your health. I will only use this information for informing treatment and associated recommendations concerning aspects of health and wellbeing which I will offer to you. On my website I take basic contact details and information via my website to allow me to contact you and handle bookings.


Lawful Basis for holding and using Client Information


As a full member of the Nursing and Midwifery Council, I abide by the NMC Code of Practice and Ethics.


The lawful basis under which I hold and use your information is my requirement to hold your information for the following legal reasons

As I hold special category data (i.e. health related information), the Additional Condition under which I hold and use this information is: for me to fulfil my role as a health care practitioner bound under the NMC Confidentiality as defined in the NMC Code of Practice and Ethics.



What information I hold and what I do with it


In order to give ear care treatments, I will need to ask for and keep information about your health. I will only use this for informing treatments and any advice I give as a result of your treatment. The information to be held is:

  • Your contact details

  • Medical history and other health-related information (which I will take from you at first consultation)

  • Treatment details and related notes (which I will take after each consultation)


I will NOT share your information with anyone else (other than within my own practice, or as required for legal process) without explaining why it is necessary and getting your explicit consent.



How Long I Retain Your Information for


I will keep your information for the following period

Adults - 8 years

Children - up until they reach 25, or if 17 when treated until they are 26 years old.

Claims Occurring Insurance - 7 years after date of last treatment.

My Insurance and the NMC require me to retain information for this length of time.


Your data will not be transferred outside the EU without your consent.


Protecting Your Personal Data


I am committed to ensuring that your personal data is secure. In order to prevent unauthorised access or disclosure, I have put in place appropriate technical, physical and managerial procedures to safeguard and secure the information we collect from you.


I will contact you using the contact preferences you give me in relation to:

  • Appointment times                                                                               

  • Information related to your health

GDPR gives you the following rights:


  • The right to be informed:
    To know how your information will be held and used (this notice).

    To see your nurses records of your personal information, so you know what is held about you and can verify it.

    To tell your nurse to make changes to your personal information if it is incorrect or incomplete.

  • The right to erasure (also called “the right to be forgotten”):
    For you to request your therapist to erase any information they hold about you

  • The right to restrict processing of personal data:.
    You have the right to request limits on how your therapist uses your personal information

  • The right to data portability: under certain circumstances you can request a copy of personal information held electronically so you can reuse it in other systems.

  • The right to object:
    To be able to tell your nurse you don’t want them to use certain parts of your information, or only to use it for certain purposes.

  • Rights in relation to automated decision-making and profiling.

  • The right to lodge a complaint with the Information Commissioner’s Office:
    To be able to complain to the ICO if you feel your details are not correct, if they are not being used in a way that you have given permission for, or if they are being stored when they don’t have to be.

SH Earcare RIGHTS

Please note:

  • if you don’t agree to your Nurse keeping records of information about you and your treatments, or if you don’t allow them to use the information in the way they need to for treatments, the nurse may not be able to treat you  

  •  Your nurse has to keep your records of treatment for a certain period  as described above, which may mean that even if you ask them to erase any details about you, they might have to keep these details until after that period has passed

  • Your nurse can move their records between their computers and IT systems, as long as your details are protected from being seen by others without your permission.
